I hereby authorize the Mike Stankovic Pro Soccer Academy to act for me according to their best judgment in any emergency requiring medical attention and hereby waive and release the Mike Stankovic Pro Soccer Academy from liability for injury orillness incurred while at camp. I verify to the best of my knowledge that the above named applicant is physically able to participate in the activities of the camp. I waive and forever discharge the Mike Stankovic Pro Soccer Academy at McDonogh High School and Garrison Forest School from any and all rights and claims for damages to persons or property while at the camp site.
